This Telehealth Informed Consent ("Consent") explains the nature, risks, and benefits of telehealth services provided through Vionti Health. By using our telehealth services, completing a health assessment, or accepting treatment prescribed through our platform, you acknowledge that you have read, understood, and agree to the terms of this Consent.
1. Nature of Telehealth Services
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. Through Vionti Health, telehealth services may include:
- Asynchronous consultations where a licensed physician reviews your health assessment questionnaire and medical history
- Synchronous (real-time) consultations via secure messaging, phone, or video when medically necessary
- Evaluation and diagnosis of your health condition
- Prescribing of compounded medications when clinically appropriate
- Follow-up care and ongoing management of your treatment
2. Benefits of Telehealth
Telehealth offers several potential benefits, including:
- Convenient access to healthcare services from the privacy of your home
- Reduced need for in-person office visits for appropriate conditions
- Timely evaluation and treatment without long wait times
- Discreet consultations for sensitive health concerns
- Access to licensed physicians regardless of geographic location (within the United States)
3. Risks and Limitations of Telehealth
As with any medical service, telehealth has certain risks and limitations that you should understand:
- Limited physical examination: Your provider cannot perform a hands-on physical examination. This may limit the provider's ability to diagnose certain conditions
- Technology limitations: Technical difficulties, including interruptions in service, poor connectivity, or equipment failures, may occur and could affect the quality of care
- Information limitations: The provider's assessment is based on the information you provide. Incomplete or inaccurate information may result in inappropriate recommendations
- Not suitable for all conditions: Some medical conditions require in-person evaluation. Your provider may determine that telehealth is not appropriate for your situation and refer you to in-person care
- Security risks: While we use encrypted, HIPAA-compliant communications, electronic communications carry inherent risks of unauthorized access
4. Patient Responsibilities
By using our telehealth services, you agree to the following responsibilities:
- Accurate information: You must provide truthful, accurate, and complete health information, including your medical history, current medications, allergies, and any other information requested by your healthcare provider. Failure to disclose relevant health information could result in adverse health consequences
- Follow instructions: You must follow all treatment instructions and medication guidelines provided by your healthcare provider and pharmacist
- Report adverse effects: You must promptly report any side effects, adverse reactions, or changes in your health condition to your healthcare provider
- In-person care: You understand that telehealth is not a substitute for in-person medical care when necessary. You agree to seek in-person care when recommended by your provider
- Primary care provider: You are encouraged to inform your primary care provider about any treatments received through Vionti Health
5. Prescription Medications
If your healthcare provider determines that medication is appropriate for your condition, you acknowledge and understand the following:
- Medications prescribed through Vionti Health are compounded by licensed U.S.-based compounding pharmacies based on your physician's prescription
- Compounded medications are not FDA-approved as finished drug products. They are, however, prepared using FDA-regulated ingredients by state-licensed pharmacies in compliance with applicable pharmacy regulations
- Available dosage forms may include sublingual tablets, chewing gum, and other formulations as determined by your prescribing physician
- All medications carry potential risks, including side effects, allergic reactions, and drug interactions. Your provider will discuss relevant risks with you
- You must disclose all medications, supplements, and substances you are currently taking to avoid potentially dangerous interactions
- Medications are prescribed for your use only and must not be shared with, sold to, or distributed to any other person
- You must follow all dosage and usage instructions exactly as prescribed
6. Right to Refuse or Withdraw
You have the right to:
- Refuse telehealth services at any time without affecting your right to future care or treatment
- Withdraw your consent at any time by discontinuing use of our Services and notifying us in writing at support@viontihealth.com
- Request an in-person consultation instead of telehealth services
- Seek a second opinion from another healthcare provider
- Decline any recommended treatment or medication
Withdrawing consent will not affect the legality of any treatment provided before your withdrawal.
7. Emergency Situations
Vionti Health telehealth services are NOT designed for medical emergencies. If you are experiencing a medical emergency, chest pain, difficulty breathing, severe allergic reaction, or any life-threatening condition, call 911 or go to your nearest emergency room immediately.
Our telehealth platform is designed for non-emergency consultations related to sexual health. In the event of an urgent or emergent situation, you should:
- Call 911 or your local emergency services immediately
- Go to the nearest hospital emergency department
- Contact the Poison Control Center at 1-800-222-1222 if you suspect a medication overdose or poisoning
8. Provider Communication
Communication between you and your healthcare provider through the Vionti Health platform may include:
- Secure messaging through our HIPAA-compliant platform
- Phone or video consultations when determined necessary by your provider
- Email communications regarding appointment reminders, treatment updates, and account information
You understand that:
- Response times may vary based on provider availability and the nature of your inquiry
- Non-urgent messages are typically responded to within one to two business days
- Secure messaging should not be used for urgent or emergency communications
- All communications are documented in your medical record
9. Privacy and Confidentiality

10. Records and Documentation
All telehealth consultations and communications are documented in your medical record. You have the right to request a copy of your medical records at any time by contacting us at support@viontihealth.com.
11. Acknowledgment
By using Vionti Health's telehealth services, you acknowledge that:
- You have read and understand this Telehealth Informed Consent
- You have had the opportunity to ask questions about telehealth services
- You understand the risks, benefits, and limitations of telehealth
- You voluntarily consent to participate in telehealth services
- You understand that your consent may be withdrawn at any time
